Tretinoin works. It also dries out skin fast if you pair it with the wrong moisturizer. Most people who quit tretinoin in the first month do not quit because tretinoin failed them. They quit because their skin got too dry, too irritated, or started stinging every time they applied anything. The moisturizer is usually the missing piece, not the tretinoin.
I am Dr. Kiran Shaheer, an aesthetic doctor based in Islamabad. In this guide, I will explain what makes a moisturizer safe to use with tretinoin, which ingredients to avoid, and which products I actually recommend to patients at my clinic. I will also cover a few things specific to Pakistan that most skincare guides never mention.
Quick Summary
- Tretinoin thins the outer skin layer for a few weeks. A moisturizer is part of the treatment, not an extra step.
- Look for ceramides, hyaluronic acid, and glycerin. Avoid AHAs, BHAs, citric acid, and fragrance.
- Apply moisturizer before tretinoin for your first month. Switch order later once skin adjusts.
- Tretinoin is sold over the counter in Pakistan under local brand names. No prescription needed.
- Hard water and heat across most of Pakistan make barrier support more important here than in cooler climates.
Why Tretinoin Needs a Good Moisturizer
Tretinoin speeds up skin cell turnover. This is why it works so well for acne, texture, and pigmentation. But it also thins the outer layer of skin for a few weeks while your skin adjusts. That outer layer is what keeps water in and irritants out. When it thins, water escapes faster than usual. This is what causes the dryness, flaking, and stinging that people associate with tretinoin.
A good moisturizer does not slow tretinoin down. It protects your skin while tretinoin does its job underneath. Skipping moisturizer does not make tretinoin work faster. It just makes the side effects worse.
A thinner outer layer means moisture escapes the skin faster than usual during adjustment.
Ceramides and hyaluronic acid rebuild what tretinoin is temporarily thinning.
Dry skin can overproduce oil to compensate, which can lead to more clogged pores.
Most skin settles into tretinoin within eight to twelve weeks of consistent use.
Dr. Kiran’s Note: I see this constantly in clinic. Someone starts a good tretinoin routine, uses whatever moisturizer was already in their bathroom, and quits in week three thinking their skin cannot handle retinoids. Usually their skin was fine. Their moisturizer was not.
What to Look for in a Tretinoin-Safe Moisturizer
Ignore the front label. Check the ingredient list instead. A few ingredients matter far more than the brand name.
Look For
- Ceramides
- Hyaluronic acid
- Glycerin
- Niacinamide
- Panthenol (pro-vitamin B5)
Avoid Pairing With
- AHAs and BHAs (glycolic, lactic, salicylic acid)
- Citric acid
- Fragrance and essential oils
- Denatured alcohol
- Vitamin C in the same product
Citric acid is a sneaky one. It shows up in many moisturizers marketed for sensitive skin, but it is still a mild acid and can react with tretinoin. If a moisturizer lists ten or more active-sounding ingredients on the front, check the back carefully. Simple formulas usually perform better here than complex ones.
Should You Apply Moisturizer Before or After Tretinoin?
Both methods work. The right one depends on how long you have been using tretinoin.
If you are new to tretinoin, apply a thin layer of moisturizer first. Wait a minute or two, then apply tretinoin on top. This is often called the sandwich method. It slows down absorption slightly, which reduces stinging without reducing your results. Tretinoin still works fine under moisturizer, as long as the moisturizer is not a thick, heavy balm.
If your skin has already adjusted after eight to twelve weeks, you can apply tretinoin first and seal it with moisturizer afterward. This gives tretinoin slightly stronger contact with your skin. Most patients only need the sandwich method for the first month.
The Pakistan Factor Most Guides Skip
Most moisturizer guides online are written for the US or UK market. In those countries, tretinoin needs a prescription. In Pakistan, that is not true. Tretinoin is sold over the counter under local brand names like A-Ret, Aleton, Actret, Tretix, and Trenoin, usually priced between Rs 80 and Rs 400 depending on strength. You do not need a prescription to buy it here.
This is useful, but it also means you are managing your own routine without a doctor automatically checking your skin type first. That makes the moisturizer step more important, not less.
Pakistan’s climate adds another layer. Much of the country has hard tap water, high heat, and heavy dust exposure for most of the year. This pulls moisture out of skin faster than in cooler, softer-water climates. A moisturizer that works fine in Europe may not be enough here.
Moisturizers I Recommend to My Patients
I stock two moisturizers at my clinic for tretinoin users, because they solve two different problems. Neither contains steroids, hydroquinone, or fragrance.
Emmo Light Moisturizing Lotion
Ceramides and hyaluronic acid, with SPF 20 built in. Lightweight, non-comedogenic, safe for acne-prone skin. Use it for both layers of the sandwich method, before and after tretinoin.

Common Mistakes to Avoid
Buying “sensitive skin” moisturizer without reading the label. Many of these still contain citric acid or fragrance. Read the ingredient list every time.
Switching moisturizers mid-adjustment. If you change two things at once, you will not know which one caused a reaction. Pick one moisturizer and stick with it through the adjustment period.
Stopping moisturizer once skin feels normal. Tretinoin keeps thinning the outer skin layer for as long as you use it. Your skin needs support the whole time.
Frequently Asked Questions
Can I use a retinol moisturizer along with tretinoin?
No. This adds a second retinoid to your routine without you realizing it. Keep your moisturizer free of active ingredients and let tretinoin be the only retinoid you use.
Why does my moisturizer burn after I apply tretinoin?
This usually means your barrier is compromised, or your moisturizer contains an ingredient like citric acid or alcohol that reacts with tretinoin. Switch to a simple, fragrance-free formula. If the stinging does not stop within a week, read my full guide on why moisturizer burns and stings.
Do I need a different moisturizer for summer and winter in Pakistan?
Often yes. A light lotion like Emmo Light works well through summer heat and humidity. Cold, dry winter air or heavy air conditioning may call for a richer cream like DERM REST, even without an active flare-up.
Is body lotion okay to use on my face while on tretinoin?
Not ideal. Body lotions are made for thicker skin and often contain fragrance or alcohol that facial skin cannot tolerate well during tretinoin use. Stick to a moisturizer made for the face.
How long do I need to be careful about moisturizer choice?
As long as you use tretinoin. Most people notice their skin becomes far less reactive after eight to twelve weeks. For the full adjustment schedule, see my complete retinoid guide.
